New research suggests a decade-long diet soda habit could carry a cognitive cost most people never see coming
For years, artificial sweeteners have been marketed as the smarter substitute — all the sweetness, none of the sugar. But a sweeping new study out of Brazil is complicating that narrative, suggesting the trade-off may not be as clean as it seems.
The Sweeteners Under Scrutiny
Researchers at the University of São Paulo tracked nearly 13,000 adults over eight years, examining how their long-term intake of low- and no-calorie sweeteners tracked against changes in memory and cognitive function. The results, published in the American medical journal Neurology, point to a consistent pattern: the more sweeteners a person consumed, the sharper their mental decline over time.
Six specific sweeteners were flagged in the findings — aspartame, saccharin, acesulfame potassium, erythritol, sorbitol and xylitol. All six are common fixtures in diet sodas, sugar-free snacks and “zero-calorie” products lining supermarket shelves.
What the Numbers Reveal
The gap between high and low consumers was striking. Participants who consumed the most sweeteners took in an average of 191 milligrams a day — roughly what’s found in a single can of diet soda. That group experienced cognitive decline 62 percent faster than participants who consumed just 20 milligrams daily, a fraction of the amount.
For researchers, the scale of that difference was hard to ignore, especially given how normalized diet soda and sugar-free products have become in daily routines.
A Health Halo Reconsidered
Dr. Claudia Kimie Suemoto, an associate professor at the University of São Paulo and one of the study’s authors, said the findings challenge a widely held assumption. Sweeteners have long carried what researchers call a “health halo” — the perception that anything replacing sugar must automatically be safer. This study suggests that halo may not hold up when it comes to long-term brain health, and that some of these substitutes could carry their own risks over time.
Why This Isn’t the Final Word
Despite the striking correlation, the study stops short of proving cause and effect. Because it’s observational — tracking people’s habits rather than controlling their diets in a lab setting — researchers can’t say definitively that sweeteners caused the cognitive decline. Other lifestyle factors, underlying health conditions, or dietary patterns that often accompany heavy sweetener use could also be playing a role.
Still, the size of the study and the consistency of the pattern across six different sweeteners give the findings weight. Researchers say it strengthens the case for more targeted, controlled studies to determine whether the link is truly causal — and if so, which sweeteners pose the greatest risk.
What It Means Going Forward
For now, the takeaway isn’t necessarily to eliminate sweeteners overnight, but to reconsider how automatically they’re reached for as a “healthier” default. As the science around long-term sweetener use continues to evolve, moderation — rather than blind substitution — may be the more prudent approach.
